Incident | A Lufthansa Boeing 787-9 Dreamliner, D-ABPQ, experienced a sudden nose gear collapse when standing at gate A15 (Terminal 1) at Frankfurt Airport (FRA/EDDF), Frankfurt, Hesse. Two cabin crew members and several other employees of service providers were injured and taken to a hospital for medical treatment. Flight DLH450 to Los Angeles International Airport (LAX/KLAX) was cancelled. There were 13 crewmembers and no passengers on board and the aircraft was taken out of service for inspection and repairs. The preliminary report states that technicians were conducting a test to resolve an issue regarding the control system for the main landing gear doors, as a part of this test the landing gear lever should be in the "UP" position. It was found during the investigation that the landing gear downlock pin for the nose landing gear was not installed and still in the storage box. |

An Air Europa Boeing 787-9, registration EC-ODH performing flight UX-57 (sched. dep Jul 9th, act. dep Jul 10th) from Madrid,SP (Spain) to Sao Paulo Guarulhos,SP (Brazil), was enroute southbound on the same airway at FL360 over the Atlantic near the West Saharan Coast. TCAS resolution advisories activated in both cockpits, the A21N descended and the B789 climbed. According to information The Aviation Herald received, both aircraft had been cleared to maintain FL360 when TCAS RA occurred at about 01:22 UTC: Both aircraft continued to their destinations for safe landings. Spain's CIAIAC opened an investigation and reported on Jul 17th 2026: "On July 10, 2026, the AIRBUS A-321 aircraft with registration EC-OLE and call sign IBE0140 was flying on airway N857 at flight level FL360 heading northeast, located between points ETIBA and BIPET in the oceanic sector of the FIR/UIR of the Canary Islands (Spain). At 01:23 UTC, a TCAS TA warning was activated, followed by another “TCAS RA DESCEND” warning, because another BOEING 787-9 aircraft with registration EC-NBM and flight call sign AEA05 was at the same flight level flying on the same airway in the opposite direction. On this other aircraft, a “TCAS RA CLIMB” alert was activated. The AIRBUS A-321 aircraft descended 500 ft following the system instructions, until it received the LEVEL OFF instruction, and the BOEING 787-9 aircraft ascended 400 ft, also following the system instructions, until it received the “CLEAR OF CONFLICT” warning. The two aircraft continued their respective flights without any injuries or personal damage." |

Accident | Engineer, 23, Dies in Tragic Boeing 787 Maintenance Accident at Heathrow Airport A 23-year-old Virgin Atlantic aircraft engineer has tragically died following an accident while carrying out maintenance on a Boeing 787 at Heathrow Airport. Georgie Buxton was reportedly conducting maintenance tests inside one of the aircraft's fuel tanks when equipment is believed to have collapsed, causing fatal injuries. The aircraft involved has been grounded while a full investigation takes place. Virgin Atlantic is working alongside the relevant authorities and Boeing to establish the circumstances surrounding the incident. At this stage, the exact cause of the accident remains unknown. Georgie joined Virgin Atlantic as an aircraft technician two years ago and was described by colleagues as a much-loved member of the engineering team. In a statement, Virgin Atlantic said: "We are heartbroken to confirm that a much-loved member of our engineering team has passed away. Our thoughts are firmly with Georgie's family, loved ones, friends and colleagues at this incredibly sad and challenging time. We're providing support to loved ones wherever we can and sharing our heartfelt condolences to everyone affected. Georgie's family has asked for privacy at this very difficult time. The safety and wellbeing of our people is always our top priority, and we are devastated by this incident. We are working closely with the relevant authorities and the aircraft manufacturer on the investigation to establish all the facts. Out of respect for those involved, and while the investigation is ongoing, we are unable to provide any further information at this stage." Heathrow Airport also expressed its condolences, saying: "We were very sorry to hear of this tragic accident. Our thoughts are with Georgie's family and friends, as well as all of our colleagues in the wider Virgin Atlantic family who feel this loss." As the investigation continues, the aviation community mourns the loss of a young engineer whose career had only just begun. MAJOR INCIDENT | Notes from AVH - On Jun 12th 2026 the AAIB released this very brief interim statement without answering any of the questions the preliminary report left open despite the relevant data already known and available via the EAFR. The statement reads in part: Over the past year, the investigation team has undertaken an extensive and rigorous examination of all relevant technical, operational, organisational and human factors associated with the accident. This effort has been supported by accredited representatives, technical advisers and subject matter experts from relevant organisations. Significant progress has been made in the examination and analysis of aircraft systems, flight recorder data, engine-related components, maintenance and operational records, and other evidence relevant to the investigation. The evidence gathered and the results of various examinations are currently being analysed in a comprehensive and integrated manner. Additional technical evaluations and specialist examinations, wherever considered necessary, will continue to be undertaken to ensure that all findings and conclusions are supported by verified evidence and sound scientific analysis. Engineering Simulator Tests conducted in an investigation by a US based lawyer representing the families of 135 victims show that after moving the fuel switches to CUTOFF it takes between 14 and 18 seconds until the RAT would be able to supply hydraulic power - the results of these tests would put the time of the RAT receiving the signal to extend to or before 08:08:33Z (V1 speed achieved) and thus way before the fuel switch signals transitioned to CUTOFF at 08:08:42Z, thus disproving the RAT deployed in response to the fuel switches. The lawyer stated: "The RAT deployment is a symptom of something else going on. In order for it to be out, something has happened … if it is out prior to the fuel switch allegation, our question still is: why? It is a symptom of something that has gone wrong." |
